Athlete or Bencher of The Year: Any OPA powerlifter who has achieved outstanding extraordinary success with OPA, Canadian Powerlifting Union, or/and as a Canadian lifter; and also demonstrates exemplary values and/or personal characteristics, and makes a defining contribution to powerlifting and/or Canadian society within last year.
Athletes: Athlete Hall of Fame: Any OPA powerlifter who has achieved outstanding & extraordinary success with OPA, Canadian Powerlifting Union, or as a Canadian lifter and also demonstrates exemplary values and/or personal characteristics, makes a defining contribution to powerlifting and/or Canadian society. Athlete nominees shall not ordinarily be considered until three (3) years after retirement from major championships and competitions. Master lifters may be considered retired.
List the major achievements and contributions of the nominated person which you feel merit consideration for election in OPA Hall of Fame.

Builders: The OPA Recognition Award: The award is presented to the OPA member who best demonstrates the qualities of "Perseverance, Sportsmanship and Dedication to the Sport of Powerlifting". Any individual who has made an outstanding and extraordinary contribution to powerlifting other than as a competitor; demonstrates exemplary values and/or personal characteristics, and makes a defining contribution to powerlifting e.g. loading spotting, meet organizing, refereeing, powerlifting organization committee member at any level and coaching (voluntary not paid coaching) Builder nominees may be either active or inactive at the time of their consideration. Minimum 10 years of contribution is required to be nominated.

Any business related or business promotional contributions may not be considered for OPA Recognition Award.
Only completed forms will be accepted by the Ontario Powerlifting Association for consideration.
NOMINATION FORMS for consideration must reach your provincial executive before the deadline given by them for their AGM
